Job Description

  • Both the intake and referral roles play crucial parts in the overall homecare process, ensuring that clients receive the right care and that communication flows smoothly between healthcare providers, clients, and caregivers
  • The referral coordinator establishes and maintains relationships with hospitals, medical facilities, physician offices, social workers, and other healthcare professionals to receive referrals for potential homecare clients
  • Upon receiving a referral, the coordinator assesses the referred individual’s needs and matches them with appropriate homecare services. This may involve reviewing medical records, speaking with medical professionals, and understanding the client’s specific requirements.
